Facility Rental Requirements

  • Insurance Requirements

    Additional Insured Endorsement must name the Millbrae School District and its Board as additional insureds and must be in the form of an actual endorsement to the policy. (Blanket endorsements will not be accepted)

    Applicant is required to upload required Insurance Certificates into Civic Permits.

    General Liability Coverage: $1,000,000.00 per occurrence, $2,000,000.00 aggregate
    Workers Compensation Coverage: $1,000,000.00 per occurrence
    Automobile Liability Coverage: $1,000,000.00 per occurrence (subject to event details)

    Facility Rental Application

    The Millbrae School District utilizes Civic Permits, an online facility request and rental website.

    1. Follow instructions to create an account
    2. Applicant must be an duly authorized representative or officer of the entity over 18 years of age
    3. The Application submitted must be for the Applicant's direct use of school facilities or grounds (no 3rd party beneficiaries are allowed)
    4. Current & Valid Insurance Certificates must be uploaded
    5. Agree to Terms and Conditions 
    6. Request should be entered at least 4 weeks prior to event
    7. All users must have an approved request on file PRIOR to using facilities
    8. Advertising is not permitted without prior approval of Superintendent or designee
