All of the following documents are required and must be provided at the time of the registration appointment or successfully emailed to the school site secretary. Please do not schedule a registration appointment until you can provide all required documentation. Missing or incomplete documents will delay the registration process. Please contact your resident school with any questions.

The McKinney-Vento Homeless Assistance Act, part of Every Student Succeeds Act (ESSA), entitles all school-aged children experiencing homelessness access to the same free, appropriate public education that is provided to non-homeless youth. Schools are required to remove barriers to enrollment, attendance, and academic success of students experiencing homelessness

Verification of student age: original birth certificate, certified copy of birth certificate, statement by local registrar or county recorder certifying the date of birth, baptism certificate duly attested, passport or other form of identification confirming the student's legal name and date of birth

IMMUNIZATION RECORDS
Please provide an original immunization card (yellow card) or an Immunization Report provided by your healthcare provider.

NEGATIVE TUBERCULOSIS RESULTS OR CALIFORNIA PEDIATRIC TUBERCULOSIS RISK ASSESSMENT
This is required if registering for the first time in a California public school, if the student is coming from out of the country or out of the state, or if this is the first time registering for Transitional Kindergarten or Kindergarten.

Final Registration Steps

When all registration documents are received by the school site, you will receive instructions to complete the registration process in our online registration portal. Please complete the online process as soon as possible. All students will be assigned to their resident school based on their home address and classroom space availability. If there is a lack of classroom space, your student will be placed on a waiting list for your resident school and will be assigned to another school within the Millbrae Elementary School District with the available classroom space.
