Volunteer Clearance Instructions
The Millbrae Elementary School District staff will request that visitors provide proof of vaccination status before entering any campus. If visitors are not vaccinated or refuse to provoide proof, they will not be permitted on any school campus until the current health order expires.
Volunteer clearance and services for the Millbrae Elementary School District are planned to resume on September 16, 2021. (subject to change based on CDPH guidance)The Millbrae Elementary School District (MESD) is committed to keeping student safety a top priority. The MESD requires that all parents who volunteer in the classroom, chaperone or drive on field trips, must be fingerprinted. Fingerprinting is the most accurate way to collect information that insures a person is permitted to participate in a school setting. You must also have Volunteer Clearance in order to be a Parent Driver. Fingerprinting is arranged by the District Office upon receipt of a completed application form and all of the required documents. Once you have been fingerprinted for the Millbrae Elementary School District you will not need to be fingerprinted again for the Millbrae Elementary School District.
Step 1: Complete the Volunteer Clearance Authorization Form (available in the school office or on our website at millbraeschooldistrict.org).
Step 2: Attach a copy of your current driver’s license.
Step 3: Attach negative TB Results (CA Education Code 49406) – negative TB results taken within the last 6 months are required or TB Risk Assessment Questionnaire certified by a licensed health care professional must be provided
Step 4: Submit paperwork to your site principal for approval.
Step 5: Site principal will forward the completed paperwork to the District Office for processing
Step 6: You will be contacted via email by the District Office to schedule a fingerprinting appointment. (If you do not have an email address, you will be contacted by phone)
***Fingerprint results can sometimes take up to two weeks to be processed. ***
The MESD may offer fingerprinting services throughout the school year. Upon receipt of approved volunteer clearance forms, you will be contacted by the District Office and given the appropriate paperwork to be fingerprinted at IAR Livescan in San Bruno. Once you have received the Livescan paperwork from the District Office, you will need to make an appointment with IAR Livescan whose operating hours are 9:00 am – 5:00 pm, Monday through Friday. The cost for fingerprinting via IAR Livescan is $72 (subject to change), payable by cash, credit/debit cards or cashier’s check. IAR will not process your fingerprinting without the required forms provided by the District Office.
Parent Driver Instructions (must be re-submitted annually)
Step 1: You must have Volunteer Clearance (follow steps above)
Step 2: Complete the Parent/Volunteer Driver Authorization Form (available on our website at millbraeschooldistrict.org).
Step 3: Attach a copy of your valid driver’s license (REQUIRED)
Step 4: Attach a copy of your automobile insurance declaration page (REQUIRED) which indicates limits of liability and policy expiration date. Your policy must provide a minimum liability limit of $100,000.00 to be permitted to drive students.
Step 5: Submit paperwork to your site principal for approval.
