Employment Application

You will need to completely fill out and submit the proper Millbrae School District employment application: Certificated Application Form if you are interested in our of our certificated positions; an Administrative Application Form is required for administrative positions or a Classified Application Form is required for all classified positions. Here are the two ways to obtain an application:

1. Download a Certificated Employment Application, Administrative Employment Application or Classified Employment Application from this website. You will need Adobe Acrobat Reader  (available free) to be able to access the applications.  Once you have downloaded the application, fill it out on-screen, print it and sign it.
   

2. Pick up an application from our Personnel Office at 555 Richmond Drive from 8 a.m. to 4:00 p.m.
    

3. MSD current classified employees: TRANSFER OR PROMOTION REQUEST FORM
