Please review the following forms and sign and return on Maze Day: August 25, 2011 for 6^th graders and August 26, 2011 for 7^th and 8^th graders. All forms must be completely filled in.

#2 Emergency Release Informatio n	Fill in the form completely. Students will only be released to those individuals who are listed on this form. (Requires two emergency contacts other than parent/guardian and parent signature).
