Please review the following forms and sign and return on Maze Day: August 19, 2010 for 6th graders and August 20, 2010 for 7th and 8th graders. All forms must be completely filled in. 

MAZE DAY SCHEDULE

#1 Taylor MAZE DAY Check – List…Please present the check-off  list to the first station (Emergency Release Information) to start “First Day Packet”  process.

 #2 Emergency Release Information … Fill in the form completely. Students will only be released to those individuals who are listed on this form. (Requires two emergency contacts other than parent/guardian and parent signature).

 #3 Parental Statement About Sex Education Instruction… Check yes or no if you wish your student to participate in the program of instruction in sex education. (Requires parent signature).
