A registration appointment and complete registration documentation will be required in order to register your student.  Please follow the 2018-2019 registration instructions below. 

Registration for new students entering the Millbrae School District and students entering Transitional Kindergarten and Kindergarten for the 2018-2019 school year will require an appointment.  Please follow the registration instructions below to successfully register your student. 

All School Site offices will re-open on Aug. 9, 2018.

Transitional Kindergarten – All students who turn five (5) years old between September 2 – Dec. 2, 2018 (inclusive) may enroll in Transitional Kindergarten for the upcoming 2018-2019 school year.  


1.  Please print and complete the 2018-2019 Student Data Sheet

3.  The following documents must be provided at the time of the registration appointment:
      ♦  Student Birth Certificate (original or certified copy)
      ♦  Parent/Guardian Identification  - Actual Driver's License or Passport
      ♦  Residency Verification - 2 documents are required that contain the Parent/Guardian Name AND address on both (please click HERE for additional Residency Verification Information).  Homeowner or                Renter/Lessee - please provide either (1) Grant Deed or (2) Property Tax Bill or (3) Original Rental or Lease Agreement AND current PG&E or Water bill (or PG&E or Water service confirmation)
      ♦  Current Immunization Records - original Immunization Card (yellowcard) or Immunization Report provided by your health provider.  Please be advised - NO SHOTS, NO SCHOOL.  Please click the                 links below for more Immunization Information:

      ♦  TB Results taken within the last 12 months (if registering for the first time in a California Public School, if the student is from  out of country or out of state, or if it is the first time registering for                               Kindergraten/Transitional Kindergarten).

      ♦  Report of Health Examination for School Entry (exam must be AFTER March 1, 2018 (For students registering for Transitional Kindergarten, Kindergarten and 1st Grade.)

      ♦  Oral Health Assessment  - California Law requires that your student reeive an oral health assessment a part of school readiness activities for Kindergarten entry (or first grade if this is your child's first year in public school.)  An oral health assessment conducted the year prior to Kindergarten, or by May of your child's kindergarten year, satisfies this requirement.

      ♦  Tdap Immunization (7th & 8th Grade Registrations only)

4.    For this appointment, you must provide the completed Student Data Sheet and all required  documentation as listed above in order to successfully begin the registration process. The on-line registration link will not be provided unless all documentation is complete.

5.   When all registration documentation has been processed, you will receive instructions to register your student(s) online.  Once you have been provided the online instructions, please register your student as soon as possible.  Your registration will not be complete until you have successfully completed the online registration.

      All students will be assigned to their resident school based on their home address and classroom space availability.  If there is a lack of classroom space, your student will be placed on a waiting list for your  resident school but, in the meantime, will be assigned to another school within the Millbrae School District with the available classroom space.
